Our professionally trained Customer Service staff is available to receive your calls and assist you with questions, service issues and placing orders during normal business hours. How to Order
Surgipath Medical Industries can accept orders via telephone or fax. Telephone orders are taken from 7:00 a.m. to 5:00 p.m. CST. Fax orders are received automatically 24 hours a day.
When ordering, please include the following:
- Your customer account number
- Your required ship to location
- The Surgipath catalog item number and product description
- The quantity required
- Your purchase order number

Standard Freight Terms
Our standard freight terms are freight prepaid (including hazardous fees where required) and added to your invoice.
Shipping
Orders will be shipped via Federal Express ground, UPS ground, or LTL truck when required. For truck shipments, Surgipath must be advised if inside delivery is required.
Hazardous Cost Information
Hazardous fees will be charged according to the delivering carrier?s policies. Both Federal Express and UPS charge hazardous fees. Please contact your sales representative or customer service to determine if the product you are ordering is classified as hazardous for shipping purposes.
Note: All of our chemicals should be handled only by qualified individuals familiar with the potential hazards of the chemicals.
Return Policy
Returned goods will only be accepted with a Return Goods Authorization (RGA) number assigned by our Customer Service Dept. Please call (800) 225-3035 to obtain an RGA number if you must return goods. Returns must be initiated within 60 days of the date of the original invoice. A 25% restocking charge will result from returns caused by customer error. Customized or special order items are not returnable.
